I think Mystery Men covered the subject of superhero recognition pretty well.  You add a pair of glasses, and there is NO way this person could be recognized, or even associated with his super alter-ego.  It is just impossible! ;-)

Depending on the person.  If it's some celebrity, a lot of mask.  If it's your everyday joe schmoe, likely not that much.
