Looks to me to be between 2 and 3 inches in diameter.  It is a bit hard to judge though.
When you insult someone by calling them a "pig" or a "dog" you aren't maligning pigs and dogs everywhere.  The same is true of any term used as an insult.

No CoH-themed tattoos, but i had been considering a Rogue or Vigilante icon, more likely Rogue.

It depends a lot on your pain tolerance, and where the tattoo is done, but they're not very painful for most people, both during and after a session.

They can. Black is generally very tame, but red can be a big irritant.
