What was the 50+ (1,2,3) level and how did one achieve it?

The 50 was 'easy', just get to that level.
The 50+ was when you performed and finished the Incarnate intro mission arcs in Ouroboros, aka, becoming an Incarnate, and unlocking the Alpha slot. You then kept on 'moving up' in the series by using Alpha slot powers, and performing more Incarnate trials to unlock the next power slots, Lore and Destiny (and from those, on upwards to 8 Incarnate abilities you would wind up having, if CoH had been allowed to progress.)
Then there were the Level and Incarnate Shifts, +1, 2 and 3, which were unlocked as you achieved various incarnate powers and tasks. They were additive, to a certain extent, but not the same.
Slotting a Rare or Very Rare Alpha slot would have achieved a +1 Level shift, where you were a 50(+1.) You could also get a Level shift by using an Ultimate inspiration, which sole purpose was to do this thing. A Level Shift applied across the board, in all aspects of the game. Also note that the Ultimate Inspiration only lasted 3 minutes, and you could only have one active at a time.
Slotting a Rare or Very Rare Lore or Destiny slot, as well as participating in an Incarnate Trial, would garner another +1 in an Incarnate Shift, each. This isn't the same as a Level Shift, as the Incarnate shift only worked in Incarnate content, such as Incarnate Trials and the revamped Dark Astoria. In these areas you could have a 50(+4) if you had the four different shifts available for 3 minutes, and then 50(+3) for the rest of the time in the Incarnate system areas/tasks.

Also please note that I never did have anything past a 50+, so I don't have practical knowledge of the system. Got sidetracked by a concept. :P
I'm not *entirely* sure of the (+4), but it reads as that's what would happen. I was totally working on getting there, but...

God, how I miss this game.
I miss my Incarnate Mastermind - those (early) Incarnate Trials were a blast learning the tactics to defeat them!
yezzz.....incarnate kept you busy crafting recipes to get rare items. It was a bit confusing to me at first, but it did work.
I often wished I could 'discard' items as it really made a mess of my purse.
